Any web design company will offer you forms as an option but few will make you
a good looking form. Most are on white background, the fields are not aligned
and you get a standard confirmation page. We firmly believe that if you make
the effort to be different, your visitors will appreciate it and your site will stand out.

A form is nothing to be taken for granted and just like the rest of the site, it represents
you and it is usually the last thing a visitor remembers seeing before leaving the site.
Make the form count and stand out. At TotalDesignZ.com, we can do this.

On this page are a few samples of forms that we have made including a few forms that were
made simply to show what can be done. Note that some are a showcase in design and do
not have to look that way at all. We can make them any way you wish.

A form is nothing without a confirmation page to match (see below).
Please note that this form (as with most of our work) is made for an
800x600 resolution but could of course be made for the resolution
of your choice.

We are showing you two versions of this form not only because of
the unusual layout but also to show the importance of a custom
confirmation page. We have also place a background image on one
of them so that it will be easier to keep track of the different versions.

Note that the information on the confirmation page has been placed in a very unique way but we have only
done this to show that it can be done. Naturally we could have presented it any way you'd like and of course,
